Third-Space Endoscopy

Pioneering Techniques for Advanced Gastrointestinal Care

What is Third-Space Endoscopy?

Third-space endoscopy is a groundbreaking, minimally invasive endoscopic technique that allows for therapeutic interventions in the submucosal space of the gastrointestinal (GI) tract. This innovative approach has revolutionized the treatment of complex GI conditions, offering a safe and effective alternative to traditional surgery.

At Central Gastro, we are proud to provide advanced third-space endoscopy procedures to treat a variety of conditions with precision and minimal discomfort.

Why is Third-Space Endoscopy Performed?

Third-space endoscopy enables the treatment of complex gastrointestinal conditions that were previously managed with invasive surgeries. Common applications include:

  1. Peroral Endoscopic Myotomy (POEM):
  • Treats achalasia and other esophageal motility disorders by cutting the muscle layers of the esophagus to improve swallowing. G-POEM treats similar stomach disorders.
  1. Endoscopic Submucosal Tunneling:
  • Used to access and treat lesions or tumors located deep in the GI wall.
  1. Gastrointestinal Perforation Repair:
  • Closes perforations or leaks in the esophagus, stomach, or intestines.
  1. Zenker’s Diverticulotomy:
  • Treats Zenker’s diverticulum by removing or reducing the pouch in the esophagus.
  1. Resection of Submucosal Tumors (SMTs):
  • Safely removes benign or malignant tumors located within the wall of the GI tract.

Conditions Treated with Third-Space Endoscopy

Third-space endoscopy is ideal for managing complex GI conditions, including:

  • Achalasia and other esophageal motility disorders
  • Zenker’s Diverticulum
  • Submucosal tumors (SMTs) in the esophagus, stomach, or intestines
  • Gastroparesis (delayed stomach emptying)
  • Gastrointestinal perforations or leaks
  • Complex strictures requiring advanced repair

How is Third-Space Endoscopy Performed?

Third-space endoscopy is performed using a flexible endoscope equipped with specialized tools, under sedation or general anesthesia for your comfort.

Steps of the Procedure:

  1. Preparation:
    • Fasting for 6–8 hours before the procedure.
    • Sedation or anesthesia is administered to ensure patient comfort.
  2. Procedure:
    • The endoscope is inserted through the mouth to access the GI tract.
    • A submucosal tunnel is created by lifting the mucosal layer to access the targeted area.
    • Therapeutic interventions such as cutting, repairing, or removing tissue are performed in the submucosal space.
    • The mucosal layer is closed after the procedure to promote healing.
  3. Recovery:
    • The procedure typically takes 1–2 hours, depending on complexity.
    • Most patients can return home the same day or after an overnight observation.

Benefits of Third-Space Endoscopy

  • Minimally Invasive: Avoids traditional open or laparoscopic surgeries.
  • Precise Treatment: Targets the exact area of concern without affecting surrounding tissue.
  • Faster Recovery: Reduced downtime and quicker return to normal activities.
  • Lower Complication Risk: Minimizes the risks associated with conventional surgery.
  • Comprehensive Approach: Combines diagnosis and treatment in a single session.

What to Expect After the Procedure

  • Dietary Guidelines: You may need to follow a liquid or soft diet for a few days.
  • Temporary Symptoms: Mild throat soreness or bloating may occur but typically resolves quickly.
  • Follow-Up Care: Regular check-ups and imaging, if necessary, to ensure successful healing.
